Please have your Covid-19 Information and Liability form with you, filled and signed.  

Please give me a call if....

-If in the past 14 days you have been in contact with a person confirmed positive for Covid-19,  I will need to contact those clients who I treated the day I treated you.  

-If you have a student in a school, with confirmed cases. And your child has been exposed.  

-if in the last 24hrs you are not feeling well.  You will not be charged for not canceling 24hr ahead. Please do not wait the day of your appointment.
